Report about a voluntary ban on credits for a minor

Dependent minor’s information

• Report here the details of your dependent minor for whom you want to set a credit ban. Fill in your last name, first name and all middle names, personal identity code and address in the fields provided. Always fill in this section.

Guardians’ information

• Fill in the last name, first name and all middle names of all guardians here. Also report the personal identity codes of all guardians. 

Set a credit ban on a minor

If you want to set a credit ban on your minor child, tick the box:

• I am setting a ban on credits (2 A)

After this, fill in the period of validity for the credit ban. 

• Valid indefinitely
• Valid for a fixed period
  • If you select a fixed period, enter the start and end dates of the credit ban on the form.

Select a reason for the credit ban. You can only select one reason.

• Risk of identity theft
• Control of personal finances
• Other reason

Edit a credit ban for a minor

If you want to edit the credit ban you have set earlier for a minor, tick the box:

• I am editing a ban on credits (2B)

For example, you can edit the period of validity of a credit ban

• Valid indefinitely
• Valid for a fixed period
  • If you want to edit the fixed term set earlier or change a credit ban in force indefinitely to a fixed-term ban, fill in the new start and end dates of the credit ban on the form.

Select a reason for the credit ban. You can only select one reason.

• Risk of identity theft
• Control of personal finances
• Other reason

Remove a credit ban for a minor

If you want to remove a credit ban, tick the following box on the form:

• I am removing a ban on credits (2C)

When you remove a credit ban, you do not need to fill in the sections:

• Validity period of the credit ban
• Reason for the credit ban

Select whether you give your consent to disclosure of information to credit information companies

Information about the credit ban for a minor is always disclosed to lenders requesting a credit register extract on them. However, as a guardian, you can choose whether information about the credit ban is also disclosed to credit information companies.

Always fill in this section when you are setting a new ban on credits or editing a valid ban on credits. You can also edit your consent only.

If you are removing a valid ban on credits, leave this section empty.

Read what giving consent means and tick the appropriate box:

• I have read the purposes of use listed in the attached instructions, and I understand that the voluntary ban on credits can be disclosed for those purposes.

After this, tick one of the following options:

• I give my consent that the information on the credit ban can be shared with credit reference agencies, which in turn can disclose the information for the purposes laid down in section 19 of the Credit Information Act.
• I do not give consent that information on the credit ban could be shared with credit reference agencies, or I cancel my previous consent.

Sign and date the form

Write the date, signatures of all guardians and their names in print on the form. Also provide a phone number where we can contact both guardians. The phone number is mandatory information. We will identify you by phone before the information you have provided becomes effective.
